What to Expect from our Topkare Hospice Team
A partnership with Topkare hospice care in San Diego means getting the best possible care for your loved one. Here's what you can expect from us:
1. Initial patient interview
During the initial patient interview, we’ll discuss your medical history, symptoms, and care preferences. We’ll also take the time to answer any questions you have about our services and what you can expect. We will schedule our first visit based on your needs and timeline to ensure a seamless start to your care.
2. Personalized treatment plan
In this step, our San Diego healthcare team will visit you for the first time to conduct a thorough assessment. During this visit, we will install any necessary equipment in your home and arrange for any required supplies or medications. Please feel free to share any special requests or questions you may have; we are here to ensure your comfort and satisfaction.
3. Meet the team.
You’ll be partnered with a dedicated primary care team that will support you throughout your hospice journey. Your team will consist of a Hospice Physician, Registered Nurse, Licensed Vocational Nurse, Medical Social Worker, Home Health Aide, and compassionate Volunteers, all working together to provide comprehensive and personalized care.
4. Regular check-ins and 24/7 availability
We will schedule regular visits tailored to your needs and preferences. Additionally, our team is available 24/7 to answer any questions and provide unscheduled visits if necessary. We are here to offer support whenever you need it.
